Job Description
The Adult Day Care Center located at Waters Place, Bronx is currently seeking a part time Registered Nurse ( RN) to join our team! The work schedule is 30 hours per week, Monday to Friday from 9 am to 3:30 pm, with an occasional Saturday. The part time RN is entitled to receive a sign-on bonus of $2500.
Job Summary: Responsible for providing and coordinating multidisciplinary health services for older adults in the Day Care Center . Works with the physician and other team members in implementation of plan of care.
Duties and Responsibilities:
+ Conducts comprehensive clinical assessments identifying the service needs of each registrant upon intake and thereafter on a quarterly basis
+ Ensures that for each registrant the findings of the Nursing evaluations, the Nursing Care plan and the recommendations for Nursing follow-up are documented in the clinical record
