Job Description
- Position: Advanced Practice Provider (APN/PA) – Concierge Medicine - Internal Medicine
- Location: 915 S. Waukegan Road, Lake Forest, IL 60045
- Full Time/Part Time: Full time, 40 hours/week
- Hours: Monday - Friday between the hours of 7am - 6pm
- Saturday rotation (approx. once a month), 8am - 12pm
- Call rotation
- Outpatient- Onsite and telehealth
