Job Description
The Advanced Practice Provider (APP) provides comprehensive primary care services to adult patients through the University of Virginia Health System’s innovative Mobile Care Unit and associated population health programs. This position supports UVA Health’s mission to improve access to high-quality care and advance health equity through innovative, community-based care delivery models.
The APP practices in collaboration with an interdisciplinary care team and contributes to Interactive Home Monitoring (IHM) and primary care bridging programs, supporting continuity of care for patients across the care continuum. The position is administratively affiliated with the Department of Population Health and professionally supported by the Center for Advanced Practice, UVA Health’s professional home for advanced practice providers.
Hours are Monday-Friday with an expectation of 32 patient-facing clinical hours weekly.
