Cross credentialing facilities: NA
Credentialing timeframe: 90 days
Start: ASAP
Length of contract: 6 months
Types of cases: PEDS + Neonates MUST MEET CASE LOG REQUIREMENTS BELOW
EMR: Epic
Clinician will be required to attend Epic training and/or test out
Requirements: Pediatric fellowship required.
Provider will be doing their own cases
For Neonate (less than 60 weeks old) credentials at CHOSA: 15 cases are needed
What are the average # of patients per day? 20-30
What are the required procedures? Ortho, ENT, general, dental, radiology, GI, neuro spine and head
