Working Days & Hours: Mon-Fri work week 4 days on day shift (ie.7am to 4.15pm) 1 day in a week must work PM Shift (i.e. 12.30pm to 9.30pm) 1 day in a month must work AM shift on Saturday and following Monday will be granted Day Off Location: Bukit Batok/AMK Job Description: Implements patient care in collaboration with the multi-disciplinary teams (MDT) and according to assessed needs and health condition(s). Facilitates admissions, transfers and discharges ensures discharge planning is done timely upon patient's admission and appropriate to condition. Follows up and evaluates patients' progress according to planned care and readiness for discharge. Collaborates with the MDT to deliver evidence-based care to patients to achieve the desired care outcomes. Monitors patient for changes in condition, recognises early signs of clinical deterioration using appropriate physiological assessments and escalates as required to enable timely implementation of interventions.
