Department: Behav Hlth Case Mgmt Program
Position is located: 23 Mill Lane - Specifically for Belfast Schools (RSU 71) as well as adults and other children in Waldo County
Work Type: Full Time
Hours Per Week: 40.00
Work Schedule: 8:00 AM to 4:30 PM
** This position is eligible for a $2,500 (minus tax) sign on bonus with work commitment. **
Summary:
This Case Management position functions as a part of a Behavioral health Home model of case management, working closely with Peer Support staff and Nurse Care Managers, to deliver care coordination services to children and/or adults. We have a competitive salary with an additional incentive payment structure, strong clinical supervision with five clinical supervisors, a unique team culture amongst co-workers and a flexible schedule with the opportunity to work four ten-hour days Responsibilities:
