Work Schedule: 8:00 AM to 4:30 PM
Summary:
Working within a multi-disciplinary team, provides comprehensive case management services to improve mental health, and in conjunction with the primary care provider, improve medical health to adults with serious and persistent mental illness and children with serious emotional disturbance.
Responsibilities:
+ Coordinates and facilitates access to preventative and health promotion services, mental health and substance abuse disorder treatment, and care coordination services
