Responsibilities Coordinate patient care and follow-up appointments. Educate patients on preventive health, screenings, and chronic disease management. Monitor patients' health status and identify care needs. Liaise with doctors, nurses, caregivers, and community partners. Assist patients in using digital health tools and home monitoring devices. Maintain accurate patient records and documentation. Requirements: Diploma in a healthcare-related field or relevant work experience. Strong communication and interpersonal skills.
