Job Description
Responsibilites:
- Support the clinical team in advising clients on financial assistance schemes, ensuring timely updates for means testing reviews and subsidy renewals
- Assist the clinical team with administrative processes to facilitate effective service delivery to clients and caregivers
- Manage the onboarding of new clients, including scheduling and rescheduling of initial assessments, processing subsidy applications and responding to rehabilitation-related enquiries
- Provide support in the initial assessment of client and caregiver needs, including basic case management under the guidance of a Case Manager or Social Worker
- Enhance clients’ and caregivers’ ability to cope by coordinating and streamlining access to relevant community services
