Support vulnerable populations as a Care Manager in the Enhanced Care Management Program. Coordinate care and advocate for clients' well-being through personalized service delivery.
In this essential role, you will focus on direct service and case management, ensuring that clients have timely access to care and resources. Collaborating with clients, families, and healthcare partners, you'll promote resilience and empowerment through effective care strategies and evidence-based practices.
Key Responsibilities:
• Lead the creation of Individual Care and Health Action Plans
• Offer services in accessible formats, including telehealth
• Connect clients with needed social supports and resources
• Advocate for clients’ healthcare needs and preferences
• Conduct outreach for engagement in ECM services
Requirements:
• Bachelor's degree in an appropriate field required
• Minimum three years of relevant experience
• Community health worker experience is a plus
