As a Care Manager RN, you will:
Day to Day Responsibilities
Coordinate and manage the care members through assessment, care planning, utilization review, and discharge coordination to ensure quality, cost-effective outcomes. EHR documentation
Key Responsibilities:Review UAS assessments, identify risk factors, and develop individualized care plans with PCPs.
