Job Description
Qualifications
- Graduate of an accredited School of Nursing.
- Currently licensed as a Registered Nurse in the State in which you are applying (or another NLC state).
- 2+ years of previous experience working in care management and/or with CKD/ESRD patients is preferred.
- Ability to take calls remotely on some nights and weekends
- Must have Home Health and/or Hospice experience.
