As a Care Partner with our Aboriginal Home Health team, you’ll support Elders through their care journey in a way that respects culture and connection to land. Working closely with families, clinical teams and community providers, you’ll develop holistic care plans, coordinate services, and ensure Elders receive safe, person-centred support that enables them to age well at home and on Country.
This is a full-time role based at Penrith, NSW working in a hybrid environment, from office/home and completing customer visits.
