Support patients and families by coordinating care, facilitating community support services, and assisting Medical Social Workers in delivering holistic patient care.
Key Responsibilities- Support Medical Social Workers in patient care coordination and case management
- Manage routine inpatient and outpatient cases independently
- Conduct financial assessments for assistance schemes
- Facilitate discharge planning and coordinate community support services
- Conduct field visits and follow-up for discharged patients
- Liaise with community partners and social service organisations
- Support administrative and operational duties within the department
- Provide guidance and support for new team members
