Family Assistance Program Case Manager
This is a part-time position (21 hours per week, in person)
This position is responsible for fielding telephone and in person service inquiries for the Rhona Fischer Family Assistance Program (FAP), entering client information into the database, and providing case management to FAP clients.
Family Assistance Program Duties:
+ Serve as point of contact for new and existing FAP clientele
+ Provide identified individuals and families in FAP with phone and in-person assistance regarding available resources to meet basic human need
+ Provide direct referrals for services within JFCS (e.g., community job board, counseling & support groups, disability services, veterans’ programs)
+ When appropriate, offer referrals to community resources and programs outside JFCS
