+ Responsible for the case management of assigned individuals.
+ Makes referrals and assists with the coordination of treatment services.
+ Helps individuals identify community agencies, housing opportunities, resources, and supports.
+ Provides assistance to residents with financial linkages and resource management
+ Coordinates educational and/or vocational skill development.
+ Helps each individual develop an informal support system, including natural supports and those requested by the individual.
+ Ensure smooth transitions and minimizes fragmentation of services.
+ Monitors, tracks, and provides support to the individual.
+ Organizes and maintains case records.
+ Assists with intake and admission.
