Case Worker/Social Worker, SAMH YouthReach
YouthReach provides psychosocial support and recovery programmes for youths (aged 12 to 21) with emotional, psychological and psychiatric issues in the community. Staff work with youths and their families to empower them to achieve their goals and live in a safe, caring, and inclusive community
Objective:To ensure continuous quality of the programme and service delivery to youth & families. To work with the youth, family & community partners to enhance family strengths and resources in order to minimize risk and improve functioning.
Client Management:- Implement intensive case management service model in working with youth and their familyConduct intake assessment for youth referred to the centre
