We are seeking a Clinical Care Partner to join our Support at Home team, delivering high-quality clinical oversight and person-centred care to older Australians living independently in the community.
This is a newly created role that combines clinical expertise with relationship-based case management, supporting some of our most vulnerable clients, many of whom have experienced homelessness, mental health challenges, alcohol and other drug (AOD) issues, and complex health needs.
