Job Summary
Full‑time Clinical Pharmacist at Delta Hospital located in Delta, BC, responsible for providing individualized pharmacotherapy, patient counselling, and collaborative medication therapy management.
Responsibilities
- Consult with physicians, nurses, and other health care providers to develop and discuss therapeutic plans and ensure appropriate documentation.
- Individualize patient medication therapy by reviewing literature, patient charts, lab results, and assessing clinical response.
- Provide patient medication counselling and educational support regarding drug therapy.
- Assist in developing drug therapy guidelines and contributing case studies and review articles to professional journals.
