In the role of Case Manager, you will oversee client intakes and develop personalized supportive care plans. You'll engage with clients to teach them life skills, support their independence, and work closely with partner organizations to ensure comprehensive care. Key Responsibilities:
• Manage caseload and develop treatment goal plans
• Educate clients on available CMHA services
• Encourage peer supports and community connections

Requirements:
• Relevant post-secondary education or equivalent experience
