South Dakota State University’s Department of Allied and Population Health is offering a position for a Community Care Coordinator within the Community Practice Innovation Center (CPIC) with SD‑START. The role is based in Brookings or Sioux Falls, South Dakota and is a 12‑month, full‑time, benefits‑eligible appointment funded through a grant through August 2028. The position is open to individuals authorized to work in the U.S.; sponsorship is not available.
Key Responsibilities
- Coordinate and support community‑based projects focused on substance use prevention, treatment, recovery, and research initiatives.
- Build and maintain relationships with clients, families, community members, organizations, and project partners through outreach, engagement, case management, and referral development.
