Community Palliative Heart Failure Nurse

To work as part of the St Helens heart failure team. To develop and facilitate a cohesive management programme for patients with end stage heart failure across St Helens. The post holder will work closely with primary care and secondary care services for heart failure and palliative care.
Main duties of the job
This role provides specialist leadership in the care of patients with end‑stage heart failure, working closely with cardiology, palliative medicine, and multidisciplinary teams across hospital and community settings.
