An indicative job plan is provided, though the exact job plan will be agreed between the NCA and the post holder on commencement.
The Northern Care Alliance provides health care both in hospital and community settings. The postholder will be based primarily in Oldham Community.
The job plan is such as to allow exposure to other parts of the service and settings which will include the provision of reciprocal support to the hospital setting to cover planned absence etc.
The post holder will provide clinical leadership, diagnosis, management and treatment for patients with specialist palliative and end of life care needs; act as an expert resource for medical, nursing and other health care professionals across the Trust footprint; and be an active champion of high‑quality palliative and end of life care ...
