South London & Maudsley NHS Foundation Trust (SLaM) is looking to recruit a Consultant Psychiatrist for the South West Focussed Support Team on 12-month secondment. This is an integrated medium to long-term support service - comprising clinical, social care and voluntary sector staff – that provides multidisciplinary assessment and support. Embedded in local communities and developing strong links to the local Primary Care Networks, the team to support people to recover, stay well, make their own choices and participate in the community.
Along with the co-consultants and senior colleagues in the South West Living Well Centre, the post holder will co-lead the team which is multi-disciplinary and comprised with both conventional and new workforce.
The team normally has 11-13 new referrals per month, among which approximately half of them are eventually accepted and transferred to the team. The team discharge about 5-10 patients per...
