Endeavor Health Services is growing, and we're looking for a Critical Time Intervention (CTI) Team Leader to guide individuals through some of the most pivotal transitions in their lives-such as moving from hospitalization to community care.

In this role, you won't just manage a team-you'll shape a program that helps people build stability, strengthen support systems, and move toward independence. You'll lead a multidisciplinary team that includes case managers, nursing staff, and peer specialists, all working together to provide coordinated, person-centered care.
