Diabetic Senior / Staff Nurse (Must be 2 Years Diabetic Management Exp) Up 6K #HJC Job Responsibilities
- Assess patients' social history, lifestyle, and dietary habits to provide tailored counselling and help patients better understand and manage their diabetes.
- Collaborate closely with the Endocrinologist to develop personalized care plans for effective diabetes management.
- Conduct comprehensive patient education on insulin injection techniques, glucose monitoring (via glucometer or continuous glucose monitoring), review of glucose profiles, and other diabetes-related counselling in both inpatient and outpatient settings.
- Support the clinic's quality improvement initiatives to achieve better clinical and process outcomes.
- Perform any other duties as assigned by the Reporting Officer.
