The role:
Co-ordinate the management of discharges within the Division. The department not only manages complex discharges, but also assists patients requiring minimal intervention.To provide support education and specialist discharge knowledge for all directorates within the Acute Services Division. This includes all grades of nursing, medical and all other members of the multi-disciplinary team.
