Elementary School Social Worker

The Phelps-Clifton Springs Central School District has an opening for an Elementary School Social Worker. The successful candidate will need to:


+ Connecting students and families with school and community-based resources;

+ Identify and promote internal use of community-based services directly and in a consultation role;

+ Providing case management for students and families requiring multiple resources;

+ Providing school-based counseling per IEP, 504 or student support plan;

+ Promote social-emotional well-being of students in classrooms, lunch groups, restorative practices, etc.;

+ Support the implementation of functional behavioral assessment and behavior intervention plans in collaboration with other mental health professionals;

+ Respond as needed in crisis situations;

+ Participate in IEP, 504 meetings, building team meetings;

+ Serve as member of Pupil Support Office Team;
