Main duties of the job
As these are community‑based roles, you will be responsible for planning your own work, responding to changes in patient conditions, prioritising care needs (including hygiene, nutrition and hydration, comfort, monitoring vitals, and more) as detailed in the personalised care plan and ensuring you escalates appropriately to senior staff within the multidisciplinary team in a timely manner.
