Make a difference as a Diabetes Educator for Peace Health, working part-time in a Day position. Support patients by enhancing their diabetes management education and skills.
This role requires a dedicated educator to develop individualized learning objectives and training plans. You’ll be responsible for delivering training, evaluating its effectiveness, and conducting follow-ups to support patient progress. Additionally, you will update educational resources, ensuring all information meets current standards of practice.
Key Responsibilities:
• Develop educational goals and methods for patient training
• Provide effective diabetes self-management education
• Create tailored follow-up plans for patients
• Evaluate educational effectiveness through assessments
• Document all aspects of patient care according to standards
Requirements:
• Bachelor’s degree in Nursing or equivalent
• One year of experience in nursing or dietetics
