Transform lives as an Evening Social Worker at the Humber River Family Health Team. This role focuses on psychotherapy and support for patients and families, working evenings to accommodate diverse needs.
As a full-time Social Worker (MSW) at HRFHT, you will provide short-term therapy, intake assessments, and collaborative care in a primary health setting. Your clinical skills, experience in psychoeducation, and commitment to trauma-informed care will play an integral part in patient wellbeing and program development.
Key Responsibilities:
• Deliver short-term therapy for anxiety, depression, and trauma
• Conduct intake assessments and personalized treatment plans
• Provide supportive counseling and crisis management
• Monitor patient progress via EMR documentation
• Facilitate therapeutic and educational group sessions
Requirements:
• Master’s degree in Social Work from an accredited institution
• Registration with the Ontario College of Social Workers
