Job Description
We are seeking an experienced Nurse Practitioner or Physician (MD/DO) to provide primary care services in assisted living facilities throughout the Albany region of New York.
Position Options:
• Part-Time: 2 3 full days per week
• Full-Time: 4 5 full days per week
• Flexible scheduling available
• Expected start within 4 6 weeks, pending credentialing and onboarding
Qualifications:
• Active New York NP or Physician license
• Active DEA registration
• Currently enrolled and credentialed with Medicare and Medicaid
• Primary care experience required
• NPs must meet New York State requirements for independent practice
Preferred:
• Geriatric experience
• Assisted living or long-term care experience
