Dignity Health Medical Group - Arizona is seeking a full-time Geriatric Nurse Practitioner to join our busy department in Prescott, Arizona
- EMR: Cerner
- This position will be Alzheimer's and Dementia Care focus.
- Project volume: 250 patients
- Office Patients: Patients are seen in the office with some off-site home visits in the surrounding area
- Typical work week (hours worked; hospital vs. clinic) - option to work 4 or 5 days per week. Working in clinic setting, as well as, off site visits throughout the community to accommodate the patient's needs. Typical patient visit load of 2 new consults a day and f/u visit
Job Requirements
- Current Board Certification
- Valid medical license
- Valid DEA certificate
- 2-5 years experience in geriatrics with focus on cognitive diseases preferred
