**Job Description**
As an integral part of Mount Sinai Health System’s Emergency Department Health Education Program, Health Educators provide a patient-centered, harm reduction approach to reach individuals living with HIV, Hepatitis C (HCV), and/or Substance Use Disorder (SUD). Health Educators work with a multidisciplinary team to coordinate services and assist patients to navigate barriers to accessing quality care and treatment. This position is a one-year role subject to grant funding for possible extension and will work primarily out of Mount Sinai Queens or Mount Sinai Brooklyn with some travel to other sites required.
**Qualifications**
+ Possess a Bachelor of Science in Public Health, Biology, Psychology, Education, or related discipline
+ Have at least 1 year of experience in clinical environment
+ Must possess knowledge of community resources and health care processes, structure, and function
