Job Description
Join our interdisciplinary team and help improve the quality of life of our participants through patient-centered care. As a Physical Therapist, you will be a key member of our care team, responsible for assessing, planning, and delivering therapy treatments that promote independence, safety, and functional ability-primarily for geriatric or frail populations.
Key Responsibilities:
️ Conduct therapy assessments
️ Develop personalized care plans in collaboration with the interdisciplinary team (IDT)
️ Deliver and document therapeutic interventions
️ Perform treatments such as active/passive exercises, muscle re-education, gait training, and activities of daily living (ADLs)
️ Apply therapeutic modalities (heat, electrical stimulation, massage, etc.)
️ Supervise Therapy Assistants and support Functional Maintenance Programs
