UPMC Family Hospice is hiring a Home Care Aide for the Pittsburgh area
Provide designated care under the direction of the primary service provider in the home, in order to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.

Responsibilities:
+ Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient?s condition to appropriate professional staff.
+ Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
+ Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
