DNA Healthcare is currently seeking a Clinical Case Manager to join a well-established home care team in Darwin. About the role
You will be working within a respected organisation that has services across acute hospitals, residential aged care sites, and community care. In this role, you’ll manage a caseload of CHSP and Home Care Package clients, coordinating services, managing budgets, onboarding new clients, and ensuring care plans are delivered effectively to support client wellbeing and independence.
