About the Role
We are seeking a compassionate and experienced Hospice Nurse Practitioner (NP) to provide face-to-face patient visits and clinical oversight throughout our North Jersey service area. This role is essential in ensuring high-quality, patient-centered hospice care while supporting compliance with regulatory requirements and collaborating closely with the interdisciplinary team.
Key Responsibilities
+ Conduct face-to-face (F2F) hospice eligibility visits in accordance with Medicare guidelines
+ Perform comprehensive patient assessments to determine continued hospice eligibility
+ Provide oversight and support to hospice teams across Bergen, Passaic, Morris, Sussex, and Essex counties
+ Collaborate with physicians, RNs, social workers, and care teams to develop and update individualized care plans
+ Ensure accurate, timely, and compliant documentation in the EMR
+ Participate in interdisciplinary team (IDT) meetings...
