Reporting to the Managing Director Hospice, the RN Case Manager, provides skilled nursing care to hospice patients in their homes while coordinating individualized care plans with patients, families, and the interdisciplinary team (IDT). You'll serve as a trusted advocate, ensuring compassionate, high-quality care throughout the hospice journey.
