The Hospitalist provides medical care for patients and facilitates transition of care at discharge from the hospital by communicating a summary of the hospital course, discharge medications, pending diagnostic studies, and follow-up instructions to the primary care provider and pertinent consultants. As a Hospitalist you will:
+ Be responsible for comprehensive patient care to assure that the health care needs of patients are being met, including attendance at Interdisciplinary or nursing rounds and daily communication with the team caring for the patient.
