Job Title :- Nurse Practitioner
Location :- Commack, NY 11725
Contract :- 3+ Months with possible extension
Key Responsibilities
• Evaluate and manage patients with chronic and complex health conditions
• Perform patient assessments and physical examinations
• Diagnose conditions and develop treatment plans
• Manage medications and prescriptions
• Provide ongoing care coordination and follow-up
• Document patient encounters accurately and timely
Requirements:
• Active Adult Nurse Practitioner license
• Strong clinical assessment and diagnostic skills
• Experience in chronic care or occupational health preferred
