- Lead full-time onsite Nurse Practitioner role supporting an innovative, clinic-based model of care for patients with end-stage kidney disease (ESKD)
- Practice at multiple dialysis centers within the Chesapeake/Norfolk area, bringing primary care directly to patients where they receive treatment
- Monday Friday schedule with no nights, weekends, or on-call duties
- Flexible round-and-go model gives you control over patient visits and clinical priorities
- Help shape a new care delivery model serve as the clinical lead and collaborate closely with nephrologists, dialysis staff, and care coordination teams
- Build meaningful long-term relationships with a stable, consistent patient panel
