Job Overview
The Behavioral Health Specialist II (LCSW) provides comprehensive social work and behavioral health services to older adults enrolled in a healthy aging and wellness program. This role supports participants with complex medical, behavioral health, and social needs through assessment, counseling, advocacy, and care coordination.
As a core member of an interdisciplinary care team, the LCSW collaborates closely with providers, caregivers, and community partners to promote independence, safety, and quality of life.
