Responsibilities:
- Provide skilled nursing care to patients in home settings
- Monitor patient health and report changes to supervising RN/Physician
- Administer medications and treatments as prescribed
- Document patient care accurately and timely
- Ensure patient safety and comfort at all times
- Communication effectively with families and care team members
Requirements:
- Active Pennsylvania LPN license in good standing
