Licensed Practical Nurse (LPN)
We are currently looking for a Licensed Practical Nurse (LPN) to join our Assisted Living and Memory Care team at Christian Care. In this role, you will provide skilled nursing care to seniors within our Senior Living Community.
The LPN will be responsible for delivering direct patient care, which includes administering medications, providing wound care, and assisting with daily activities, all under the supervision of the RN Case Manager or Director of Nursing.
Responsibilities
- Prepare and update, as required, all resident service plans.
- Perform assessments on prospective residents to determine the level of care for assisted living services or memory care.
- Oversee the medication delivery plan for each resident, including self-administration and medication monitoring, medication ordering, physician orders, and recapping MARs to the physician's orders.
