This is a 100% Mobile position that requires our LVN to drive throughout the South LA and Central LA areas with areas including but not limited to SAN GABRIEL, BELL, SOUTH GATE, LONG BEACH, WEST HOLLYWOOD, LINCOLN HEIGHTS, SAN PEDRO, MONTEBELLO etc. Compass Palliative Care is looking for a professional, driven, and experienced Licensed Vocational Nurse (LVN) to see patients in their homes and provide the following care:
- Monitor and report any changes in clients' condition to the appropriate healthcare professionals. Document all care provided and maintain accurate medical records. Follow established care plans and assist with implementing therapeutic interventions. Provide emotional support and companionship to clients.
