The Role
ECC Adult Social Care, through effective practice, continues in its transformational shift from a focus on long-term care support to a model with four key principles: prevention, early intervention, enablement and safeguarding. This enables people, their families and carers to access information and tools to live healthily and independently, responding proactively to growing service demand within the Council’s financial constraints.
Working to support adults and their families who have complex social needs and are more vulnerable due to disability, exclusion or diminished capacity, you will maximise their potential to live independently. You will manage a caseload, including assessment of risk and purposeful intervention through direct work, commissioned services and review.
