- Clinic: 7a-3:30p
- Sept 20, 27
- Oct 2-3, 5-6, 11-12, 18-19, 24-26
- Nov 2-3, 6, 8-9, 15, 20, 22, 30
- Dec 7, 11, 13-14, 18, 20-21
- On Call During the week: 7a-3:30p and Night call 3:30p-7a
- Sept 15
- Oct 2-4, 6
- Dec 26-27
- On Call Weekends: 7a-7a - 24 hour call
- Sept 16 (7a) - 18 (7a)
- AUGUST-DECEMBER: OPEN AVAILABILITY (Please state your providers availability at name clear).
- Clinic: 7a-3:30p
- On call (during the week): 7a-3:30p and night call 3:30p-7a - Night call must include 2.5 hours gratis...
