The post holder will be required to:
Work as part of the existing multidisciplinary specialist palliative care team at Meadow House Hospice.Take responsibility regarding referral of patients to the service in conjunction with the multidisciplinary team.Participate in patient reviews in all settings including domiciliary visits.Act as a resource and support to all members of the multidisciplinary team involved in patient care.Ensure that all records are kept in accordance with the Trust’s record keeping policy and in line with General Medical Council’s Good Medical Practice guidance.Use electronic systems (Cerner and System1) for medical documentation.Provide clinical supervision for trainees working on the inpatient unit and will liaise closely with the Educational and Clinical Supervisors of these colleagues, with the aim of a...
