RATES:

Malpractice
Must be included in rates.

Orientation
***-142.50 - half the hourly/callback rate*** kicks in after 8 hours. Per hour of
orientation or training beyond 8 hours

Hourly ***-285 - Per hour, not OT

Vacation ***-427.50 - Must be 1.5x the hourly rate*** Per vacation hour, no OT

Nightly Call
***-800 - Per night call to hold the pager, to include 2.5 hours gratis

Vacation Nightly Call - ***-1200 - "Per night call to hold the pager, to include 2.5 hours
gratis
